Points must be reported on or before June 30, 2026
All points must be earned and in the Personify Health portal on or before June 30,
2026. There will be no run-out reporting period allowing points to be reported until
September 30, 2026. Review our wellness webpage for details.
Updates to Teladoc Program
We have updated our Teladoc points to encourage Teladoc participation throughout the year by measuring 10 times over the course of 10 weeks for diabetes, hypertension, and/or weight management.
- Remember - Teladoc is a free program offered to support those with pre-diabetes, diabetes, hypertension, and weight management. This program is available to members on a UA Choice medical plan who meet the medical qualifications to participate. Reach out to Teladoc to check your eligibility today.
Updates to the telephonic coaching
- Complete 3 coaching sessions to earn 1 point; maximum earn of 2 points
Complete Registration - Lifetime point
- This point can only be earned 1 time during a members first year in the program. It is not possible to earn this point more than once.
The FY25 Plan Year (July 1, 2024 - June 30, 2025)
- Plan year: July 1, 2024 - June 30, 2025
- Last year to report specific points until September 30
- Confirm Your Rebate
To confirm you have completed the program, log into Personify Health and click on "Rewards" under the "Ï㽶ÊÓƵ" tab at the top of the page. Then click on "Earnings History" under the gray box. Navigate to the plan year from October 1, 2024 - June 30, 2025. If you see at least 8 points (any 8 points) you have completed the program.
The FY26 Plan Year (July 1, 2025 - June 30, 2026)
- Plan year: July 1, 2025 - June 20, 2026
- All points must be completed and reported in your Personify Health platform on or before June 30, 2026
- Late points will not be accepted
Point Total
Employees need to complete 8 points to be eligible for the rebate: 2 required and 6 additional
FY25 Rebate (November 2025)
- The FY25 rebate will be paid out in November of 2025 to those who met all the rebate qualifications.
- As a reminder, you must
- (1) complete the program by June 30, 2025;
- (2) be an active employee at the time the rebate is paid out; and
- (3) be enrolled in a Ï㽶ÊÓƵChoice medical plan.
- The FY25 rebate is for the plan year July 1, 2024 - June 30, 2025

Complete the program by June 30 each year
- Complete the program and report all points on or before June 30 each year
- The program includes the required Health Check Survey, required biometric screening, and 6 additional points
- When the rebate is paid out, you must...
- be a current, benefit-eligible employee
- be enrolled in a Ï㽶ÊÓƵChoice Medical Plan
- The FY26 rebate is for the plan year July 1, 2025 - June 30, 2026
