What is Livongo - Powered by Teladoc?
Livongo - Powered by Teladoc is your chronic condition management program. This program
provides free medical supplies and support for members who qualify with pre-diabetes,
diabetes, hypertension, and/or weight management needs. The program is free to all
members enrolled in a Ï㽶ÊÓƵChoice Plan (both employees and any covered family members).
You can be eligible and participate in one or more of these chronic condition management
programs at the same time at no cost to you.
