COVID-19 Vaccine Attestation Available in Ï㽶ÊÓƵWellness Program

The COVID-19 Vaccine Attestation is now available on your PreventionCloud dashboard. If you have completed the COVID vaccine series (or received the single Johnson & Johnson vaccine), you can earn 1 point toward your $600 wellness rebate and the high point raffle. 

HOW TO EARN YOUR POINT

  • Login to your PreventionCloud account
    • Employees may login using their Ï㽶ÊÓƵcredentials at /hr/benefits/health/wellness.php
    • Spouses/FIPs may login using their PreventionCloud credentials 
      • Username: FIRSTNAMELASTNAMEBirthyear
      • Password: Birthdate in the format MMDDYYYY
  • Click "Activity Forms" on the left-hand side of your dashboard
  • Select "Submit Forms"
  • From the drop-down menu provided, select "COVID-19 Vaccine Attestation." 
  • Fill out the resulting drop down menus, including the date of the last vaccine in your series.
  • Click "Submit."
